Inaccurate filling of the form will result in delays.

Registering can take up to 15 minutes, and must be completed in one session.

You can register yourself or on behalf of a dependant in your care. 

We will contact you to confirm registration within 10 working days and to book a “New Patient Health Check” if needed.

If you have problems with internet access, ask at your local library about booking a slot to use a computer.
