Register with the Surgery

PLEASE NOTE: the registration process takes AT LEAST 5 days to complete (and often longer during busier periods. IF you do not hear back within 28 days please contact the practice).

To register with the surgery please fill out an online registration form by clicking HERE

  • When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
  • If you need assistance with registration please visit our reception from 2:00pm to 4:00pm

All patients are registered with the surgery but are allocated a named GP who is a partner of the surgery. Patients are not restricted to book appointments with their allocated GP and can book appointments with any GP in the team.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

Named GP

All practices are required to provide all patients with a named GP who is responsible for your overall care at the practice. Patients should contact the practice if they wish to know who this is, and if they have a preference as to which GP that you are allocated to. the Practice will make reasonable efforts to accommodate this request.

Whilst this doctor has been designated as having overall responsibility for care and support that our surgery provides for you, this does not prevent you from seeing any other GP or clinician in the practice as you currently do.

Accessing Someone Else's Information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
