Get a sick note for work

 

1. Log in to your NHS account on your computer or smartphone.
2. Click on Appointments.
3. Click on Additional GP services.
    Answer questions online and get a response from your GP surgery within 2 working days.

 

To request a sick note if you were more that 7 days off sick
  • Use an online form Monday to Friday, 08:00 – 18:30 to request a sick note. We will respond within 2 working days when we are open. We are open Monday to Friday, 09:00 to 18:30.
  • Phone us Monday to Friday, 09:00 to 18:30 only for inquiries. Our receptionists will not accept sick note requests over the phone with no exceptions as this can lead to mistakes.
  • Visit the surgery to request a sick note. Print, complete and place your request form in the black box outside the surgery entrance. You can also post your request to us. We are open Monday to Friday, 09:00 to 18:30.
  • Use the NHS App to request a sick note.
The Doctor may call you or arrange for an appointment. If you have already been seen by your Doctor about your illness, your sick note will be ready within 2 working days. We will send a sick note to you via SMS.

 

To request a sick note if you were 7 days off sick or less
  • Use the form on the government website to ask your employer for Statuary Sick Pay (SSP).
